HOW TO MAKE AN APPOINTMENT

You must have an appointment to be seen by a doctor or dentist at the Clinic.

The appointment line is only answered on Tuesday and Thursday mornings between 10:00am – 12:00pm for that evening’s Clinic. Appointments are taken at this time on Tuesday for general medical for that night and on Thursday for general medical and general dental (tooth extractions) for that night. All other apointments are made in person.

Eligibility Screening

Eligibility Screening Times:

Monday Evenings:
4:00 pm to 6:00 pm.

Tuesday and Thursday Evenings:
3:00 pm to 5:00 pm.

Patients must have the following qualifications:

• Resident of Lafayette Parish
• Working or actively seeking employment
• Have no health insurance
• Low to moderate income.Total income for all members of the household must not
  exceed 200% of the Federal Poverty Guidelines.

Patients Not Eligible Have:

• Medical Insurance, Medicare, Medicaid, or Disability benefits
• Job-related injuries involving Workers Compensation
• A pending medical-based lawsuit
• Not working

DOCUMENTATION NEEDED

• Most recent pay stub (If self employed,
  ledger for past three months)
• Proof of income for all members of household
  (including child support, alimony, SSI)
• Most recent 1040 income tax return
• Proof of residence in Lafayette Parish
  (utility bill or other official mail with name and
  Lafayette Parish address)
• Photo ID
• Social Security numbers of all household members.
